Chronic health conditions such as diabetes, hypertension, depression, and cancer are significant public health concerns in our state, contributing to higher healthcare costs and reduced quality of life for those affected. ​

With a legacy of creating effective programming for people of all ages, an extensive network of facilities, and expertise in evidence-based health interventions, the Y is uniquely positioned to address chronic health challenges and improve community health outcomes. YMCA health programs address major risk factors for chronic disease, by helping individuals and families of all ages to maintain a healthy weight, increase physical activity, build social connection with their peers, and overall establish healthy habits that protect wellbeing over a lifetime. ​

Our programs follow evidence-based models associated with long-term health benefits and reduced risk for chronic disease. Participants get connected to programming through a self-referral or via a referral from their healthcare provider or health plan.

Since 2016, the Y has integrated electronic referral orders for into ​dozens of clinical health systems statewide, making it easy for ​clinicians to connect at-risk patients to YMCA programs that support healthy lifestyles and social connection. Our programs are available in different languages and formats, and are tailored for cultural context to be as accessible and effective for all as possible. ​
