Foster Care Transitions

We offer a variety of programs that serve young people as they age out of the foster care system. Staff work alongside young people to help them find and maintain stable housing, healthy relationships, steady employment, educational support, and life skills. 

Range of Programs

Programs include:

  • ILP (Independent Living Program) – Assistance for foster youths’ transition to adulthood.
  • SETuP (Supplemental Education and Transition Program) - Education transition planning to help improve high school graduation rates and college enrollment.
  • IYHP (Independent Youth Housing Program) - Housing options for young adults who have aged out of the foster care system.
  • FUP & FYI (Family Unification Program & Foster Youth to Independence) - Vouchers for rental assistance.
  • WISe (Wraparound with Intensive Services) - Intensive behavioral and mental health services in home and community settings.
