Blood Pressure Self-Monitoring Virtual Group Program

This 16-week virtual group-based program, designed for adults 18 and older that have been diagnosed with hypertension (high blood pressure), helps participants establish consistent routines for monitoring their blood pressure and learn about lifestyle choices and healthy eating habits that can support you in reaching your heart health goals. Group participants will come together on Microsoft Teams every other week with a Healthy Heart Ambassador to discuss effective and accurate blood pressure monitoring, learn how to make choices to help them manage their blood pressure and set individualized goals. Group participants will also have access to free virtual healthy cooking demonstrations each week.
